Position Description

A healthcare company on Long Island is currently looking to add a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join one of their private practices team in Suffolk County.


About the Opportunity:

  • Setting: Ambulatory
  • Schedule: Monday to Friday (1 Saturday per month)
  • Hours: 8am to 5pm (1-hour break)

Responsibilities:

  • Supervise and manage Nurse Practitioners (NPs) assigned to their location
  • Complete direct reports and annual performance appraisal
  • Perform annual wellness assessments on all assigned site participants
  • Provide urgent care services to participants and facilitate the provision of same-day access
  • Assess participants upon return from the hospital/Emergency Department within 5 days
  • Review the patient's past medical history at least every 6 months
  • Formulate a comprehensive and complete diagnostic list of current and past medical conditions using clinical knowledge and judgment and the findings of his/her assessment
  • Ensure that all such documentation is complete and accurate, and specific diagnosis codes will be documented in the clinical platform and as encounter data as required

  • Review the participant's current symptoms and exacerbation of problems that were previously controlled
  • Perform other duties, as needed

Qualifications:

  • Graduate of an accredited Nurse Practitioner (NP) and/or Physician Assistant (PA) program
  • NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license
  • Computer savvy
  • Microsoft Office proficient

Desired Skills:

  • Gastrointestinal and/or Internal Medicine background
