Position Description

A health services network in New York City is actively seeking a licensed Registered Nurse (RN) to join their growing Assertive Community Treatment (ACT) team in the Bronx.

About the Opportunity:

  • Assignment Length: 13-week assignment
  • Schedule: Monday to Friday
  • Hours: 8:30am to 5pm
  • Setting: Field / Community Care
  • Team: Assertive Community Treatment (ACT)

Responsibilities:

  • Provide comprehensive, community-based nursing care to individuals with severe mental illness, often in their homes or community settings
  • Oversee medication administration, monitor for side effects, ensure proper storage and inventory, and educate clients on their prescribed treatments
  • Conduct daily health assessments to identify needs, concerns, and changes in physical or mental health
  • Develop and implement individual nursing care plans that integrate physical and mental health services
  • Provide education to clients and their families on their conditions, medications, and strategies for health promotion and wellness
  • Work closely with other members of the ACT team, including psychiatrists, social workers, and case managers, to ensure coordinated and holistic care
  • Participate in group dynamic sessions and multidisciplinary team conferences
  • Perform other duties, as needed

Qualifications:

  • 2+ years of experience in a Psych role/setting
  • Associate's and/or Bachelor's Degree
  • NYS Registered Nurse (RN) license
  • BLS certification

Desired Skills:

  • Previous experience working on an Assertive Community Treatment (ACT) team
