Case Management Nurse New

Job Title: Case Manager RN

Job Type: Contract

Location: Bronx, NY

Hours: 8:00 AM - 5:00 PM (Monday - Friday)

Primary Responsibilities:

• Ensure accurate and complete documentation of patient care and clinical activities
• Collaborate effectively with the interdisciplinary care team
• Support providers with care coordination activities
• Assist with benefit determination related to Medicare Part A, B, and D
• Partner with APCs and facilities to identify and respond to changes in patient condition
• Coordinate therapy services and participate in facility-based meetings
• Schedule and attend family conferences, team meetings, and case presentations
• Support training, tracking, and compliance with quality measures
• Develop and deliver in-services for selected audiences
• Assist providers with documentation, coding, medication reconciliation, nursing rounds, assessments, and patient records
• Write verbal orders from providers in compliance with state RN practice laws and facility protocols
• Manage members through acute medical situations or chronic condition exacerbations
• Support members during short-term sub-acute rehabilitation episodes

Required Qualifications:

• Active and unrestricted RN license in the state of residence (or ability to obtain by start date)
• Associate degree or higher in Nursing
• Certified in Basic Life Support (BLS)
• Ability to lift up to 30 pounds and perform physical assessments
• Reliable transportation
• Fully vaccinated against COVID-19 (proof required prior to employment)
• Must pass a drug screening (UnitedHealth Group is a drug-free workplace)

Preferred Qualifications:

• Minimum 1 year of clinical experience (long-term care setting preferred)
• Experience with patients who have complex behavioral, social, or functional needs
• Knowledge of geriatrics and chronic illness care
• Proficiency in documenting medical information in EMRs
• Strong communication and time management skills
• Ability to work with diverse care teams across various settings
• Willingness and ability to travel between facilities
