A Large And Growing Hospital/Hospice In Bronx, NY Is Currently Seeking An Experienced Hospice Intake Coordinator Registered Nurse, To Join Their Team! Check Out The Details Below

Title: Hospice Intake Coordinator RN

Job Type: Permanent / Direct Hire

Specialty: Field / Hospice

Location: Bronx, NY

Schedule: Full Time, Monday-Friday, 9AM-5PM

Pay: $54.65 - $68.71/hr based on experience and education

The Hospice Intake Coordinator RN will be responsible for the following duties:

  • Coordinate Agency intake, participates in the planning, implementation, documentation and coordination of patient care services
  • Coordinate intake for home care/hospice, explain eligibility criteria/reimbursement sources, and discharge planning conferences
  • Serve as educational liaison concerning range of hospice/home health services and help develop policies/procedures for intakes, and prepare all intake documentation
  • Required to commute to visit patients homes, typically 5 visits per day

Requirements

  • Active and Clean RN license in the state of New York
  • 2 years of ED, ICU, or MedSurg experience
  • Previous hospice experience highly preferred
  • At least 2 years experience working in acute care
