Pay Rate: $70-73/hr

Shift: Monday - Friday, 9:00 AM - 5:00 PM (60-minute unpaid lunch)

Days: Full-time, 5 days a week

About the Role:

We are seeking an experienced and compassionate RN Case Manager to join our integrated healthcare team in Brooklyn, NY. In this role, you will be responsible for managing utilization and discharge planning. You will assess patient needs, develop discharge plans, and ensure safe and efficient transitions for patients. You will work across various units, including medical, surgical, and emergency departments.

Key Responsibilities:

Perform utilization management and discharge planning to assess patient needs

Develop individualized discharge plans and facilitate services for smooth transitions

Collaborate with healthcare teams to ensure the best outcomes for patients

Maintain accurate and up-to-date patient records

Provide support to patients and families through the discharge process

Requirements:

• 2-5 years of experience in med-surg nursing
• 2+ years of experience in case management
• BSN required
• BLS, PRI certifications required
• Nurse to patient ratio: 1:17-20
• Unit size and census may vary
• No holiday or on-call requirements
